How KilgourMD Shifted Its Money-Back Guarantee to Onward's Checkout+
A dermatologist-founded scalp health brand stopped paying for its 90-day money-back guarantee out of margin. It moved the guarantee into Checkout+, gave customers more coverage and CashBack rewards on top, and kept the promise that gets skeptical buyers to commit to a months-long regimen.

A Clinical Hair Brand Growing at DTC Speed
KilgourMD makes scalp serums for thinning hair. Dr. James Kilgour, a Stanford-trained, board-certified dermatologist, launched the brand in January 2024. His argument is that an aging, inflamed scalp sits underneath most hair thinning, so the brand treats the scalp first. The core line is a two-step system of Treatment and Prevention serums, built on bioactives such as Redensyl, Capixyl, and Procapil. A shampoo and conditioner came later.
The brand grew fast. KilgourMD reports passing mid-eight figures in annual recurring revenue within about 18 months of launch and selling a bottle of serum every 20 seconds. It closed a Series A led by Prelude Growth Partners in 2026. Nearly all of that volume runs through kilgourmd.com, so the brand owns the full customer experience from checkout to the next order.
That ownership comes with a particular problem. Customers pay up front for a regimen, and they don't see new hair for months.
The Challenge
Hair Growth Takes Months, and Doubt Sets In Sooner
Hair regrowth is slow. In KilgourMD's own clinical work, shedding fell within about six weeks, and most participants reached meaningful regrowth around the four-month mark. That's why the brand's best-selling offers are multi-month bundles, like the 3-month supply. It also means customers commit before they've seen any change on their own scalp.
KilgourMD answered that with a promise. After a full 90-day trial, a customer who isn't seeing results can get a full refund. The guarantee does its job. It turns a hard sell, a three-month bundle for a product that works slowly, into an easy yes.
KilgourMD funded every refund itself. Each customer who used the guarantee took the full order value back out of the brand's margin, on an order it had already paid to acquire. The more the brand grew, the more the guarantee cost, and the guarantee was too central to the offer to drop.
The usual tools don't fix this. A shipping protection app covers the box and ignores the product. A stricter refund policy protects margin and undercuts the reason customers trust the brand in the first place.
The Solution
Moving the Guarantee Into Checkout+
KilgourMD launched Onward in June 2026 and moved its money-back guarantee into Checkout+, Onward's order upgrade at checkout. Customers who add the upgrade get the guarantee along with coverage for lost, stolen, and damaged shipments. They also earn CashBack rewards, store credit toward their next order. Customers get more than the guarantee alone, and the brand no longer pays for the guarantee out of margin.
The economics are simple. The upgrade generates revenue on every order that takes it, and that revenue funds the coverage, including satisfaction refunds. The protection carries no SaaS fee for the brand.
When a customer asks for their money back, or a package arrives crushed or never arrives, they file a claim through Onward's claims workflow, and Onward handles the resolution. KilgourMD doesn't have to run its own refund queue or decide case by case which requests to honor. Onward's fraud detection tools screen claims as they come in, so a generous guarantee doesn't become an easy refund for anyone who asks.
What the Claims Data Shows
KilgourMD's claims look different from most brands' claims.
Since launch, 63% of KilgourMD's claims have come from customers who weren't satisfied. Shipping problems make up most of the rest: 13% damaged in transit, 8.4% stuck in transit, plus a small number of stolen, missing, and wrong-item cases. For a typical apparel or home brand, shipping problems dominate. For KilgourMD, the biggest post-purchase risk is a customer losing confidence in the product. That's the risk the guarantee exists to cover, and the one KilgourMD used to pay for itself.
The claims cluster where you'd expect. The 3-month bundle, the brand's biggest commitment purchase, accounts for more claims than any other product, about 43% of the total, and 72% of those were satisfaction claims. The bundles that ask for the most patience produce the most "this isn't working for me" requests. Those are exactly the refunds that now run through Checkout+ instead of KilgourMD's P&L.
Volume has stayed manageable. The claims rate is 2.1% of upgraded orders. That's low enough to keep the program economical, and it shows the guarantee is doing real work for real customers.
The Results
A Guarantee That No Longer Erodes Margin
Since launching in June 2026, KilgourMD has upgraded about 11,000 orders with Checkout+. Every one of those orders carries the money-back guarantee, shipping coverage, and CashBack, and KilgourMD funds none of it from margin.
The change shows up in three places:
First-order profitability. A satisfaction refund on an upgraded order no longer comes out of KilgourMD's margin. The brand keeps the promise that sells the 3-month bundle without paying for it on every order.
Operations. Refund requests and shipping problems go through Onward's claims workflow, with fraud screening built in, instead of a support team deciding each case by hand.
The customer offer. Customers who upgrade get more than the guarantee: shipping protection on a product they're counting on, and CashBack credit toward their next supply.
How the Guarantee and CashBack Build Customer Lifetime Value
For KilgourMD, lifetime value depends on getting customers through the first few months. Shedding slows around week six, and visible regrowth comes around month four, so a customer has to stay on the regimen through at least one full bundle before seeing why it was worth it. The second order is where lifetime value starts to build.
The guarantee gets customers to start. A 90-day money-back promise is what makes a 3-month bundle an easy decision for a skeptical first-time buyer. A longer first commitment keeps the customer on the regimen long enough to see a change. When it doesn't work out, the refund is fast and handled, so the customer leaves without a fight with support.
Funding it through Checkout+ protects first-order margin. First-order profitability decides how much a brand can afford to spend to acquire the next customer. When refunds stop coming out of margin, every first order is worth more, and KilgourMD keeps more of the value of customers it has already paid to acquire. That improves LTV:CAC.
CashBack sets up the second order. Every upgraded order earns store credit that can only be spent at KilgourMD. That credit sits in the customer's account as they work through their first bundle. It lowers the cost of reordering right when the customer decides whether to keep going. For a regimen product, that's the decision that matters most.
Retention without the usual retention cost. Brands usually pay for retention with discounts, win-back campaigns, or margin given up on refunds. Here, Checkout+ revenue pays for both the guarantee and the CashBack credit, so none of that cost comes out of contribution margin.
Four months is too early to measure a full repeat cohort. The pieces that drive lifetime value are already in place: a guarantee that gets customers started, a claims process that handles problems quickly, and credit waiting at the point of reorder.
Takeaways for Premium DTC Brands
Match protection to the real risk. For a clinical or results-driven product, the main post-purchase risk is often a customer who loses faith, not a lost box. KilgourMD's claims mix (63% satisfaction) shows why order protection for DTC brands should cover satisfaction as well as shipping.
Stop paying for your guarantee out of margin. A money-back guarantee sells, and it also costs a full order value every time a customer uses it. Moving it into Checkout+ keeps the promise and takes the cost off the P&L.
Measure post-purchase by lifetime value. A guarantee and CashBack can look like a cost center and a promotion. For a regimen product with a months-long payoff, they decide whether a first-order customer becomes a repeat customer. Judge them by customer lifetime value, not by the cost of each claim.
